JET Application Required Documents
This is a general overview of the application required documents. Your country’s application will differ depending on the documents needed by your consulate/embassy. Much of the process has moved to online submission. Remember to submit all documents by the stated deadlines from your consulate!
Online Portal Submissions
4 Online Required Documents for US JETs:
- Application Form
General information, resume-style document - Self-Report of Medical Conditions
About your medical history/conditions - Statement of Purpose
Essay explaining your reason for applying (two pages, double-spaced) - Two Letters of Recommendation
May be from academic or professional references
Mail Submissions
3 Mail Required Documents for US JETs:
- Official College/University Transcripts
All institutions attended, even abroad - Proof of Graduation or Expected Graduation
Copy of diploma or transcripts, showing date of commencement - Proof of US Citizenship
Passport, birth certificate, or naturalization documents
Supplemental Materials
- Statement of Physician – Required if consulting a physician, taking medications, undergoing hospital treatment, or answered yes to questions 1, 2a, 2b, or 3.
- Certificate of Health – Form completed by a physician.
- Criminal History Background Check – FBI background check (submitted at the time of interview).
- Teaching Certification – Any state-issued teaching certifications.
- TESL/TOEFL Certification – Any certificate for teaching English as a second language.
- JLPT (Japanese Language Proficiency Test) Certification – If you have passed any level of the JLPT include the certificate.
