JET Application Required Documents

This is a general overview of the application required documents. Your country’s application will differ depending on the documents needed by your consulate/embassy. Much of the process has moved to online submission. Remember to submit all documents by the stated deadlines from your consulate!

Online Portal Submissions

4 Online Required Documents for US JETs:

  1. Application Form
    General information, resume-style document
  2. Self-Report of Medical Conditions
    About your medical history/conditions
  3. Statement of Purpose
    Essay explaining your reason for applying (two pages, double-spaced)
  4. Two Letters of Recommendation
    May be from academic or professional references

Mail Submissions

3 Mail Required Documents for US JETs:

  1. Official College/University Transcripts
    All institutions attended, even abroad
  2. Proof of Graduation or Expected Graduation
    Copy of diploma or transcripts, showing date of commencement
  3. Proof of US Citizenship
    Passport, birth certificate, or naturalization documents

Supplemental Materials

  1. Statement of Physician – Required if consulting a physician, taking medications, undergoing hospital treatment, or answered yes to questions 1, 2a, 2b, or 3.
  2. Certificate of Health – Form completed by a physician.
  3. Criminal History Background Check – FBI background check  (submitted at the time of interview).
  4. Teaching Certification – Any state-issued teaching certifications.
  5. TESL/TOEFL Certification – Any certificate for teaching English as a second language.
  6. JLPT (Japanese Language Proficiency Test) Certification – If you have passed any level of the JLPT include the certificate.
